This form is intended for members who wish to report a potential grievance. Please be aware that submitting this form does not guarantee that a grievance will be filed. All submissions will be carefully reviewed by the Unit Vice President and the grievance committee.
To assist us in evaluating your concern thoroughly, please provide as much detail as possible. Wherever applicable, include screenshots or any supporting documentation to help illustrate the issue.
